Please Print the form and return via the options on Page 7. If you choose to return the completed Distribution Request Form via email, please ensure the email is secure/encrypted to protect your personal information. You may also return your completed form to your HR department, and they will forward your form to us.
We are working to update all beneficiary information. If you have not updated your Beneficiaries after 8/1/2019 Please complete the beneficiary form. Print and return via one of the methods in Section D of the form. Your Beneficiary Designation will apply to your entire vested balance under the Plan, including the profit-sharing plan.
